Relief Pharmacy Settings in Central Florida: How Community, Health Center, LTC, Infusion and Hospital Assignments Differ

"Per-diem pharmacist" describes how you are engaged, not what you will be doing. A relief day in an independent community pharmacy and a relief day in a closed-door long-term-care operation share a licence requirement and almost nothing else.

Across Central Florida — the Orlando metro, the I-4 corridor through Polk County, the Lake County towns and the Heartland — the assignments we staff fall into a handful of settings. Knowing which one suits you is the difference between a day you would repeat and a day you would not.

Independent and discount community pharmacy

You are the pharmacist on duty, often with a small technician staff, and the work is everything at once: verification, counselling at the window, insurance rejections, prior-authorisation chasing, prescriber calls, immunisations if you are certified and the site offers them, and whatever walks through the door.

Suits you if you like autonomy and variety and are comfortable being the final answer on the floor. Ask about technician coverage on the day, the dispensing system, and whether the owner or PIC will be present.

Community health centre pharmacy

Health centre pharmacies serve patients for whom cost can be the deciding factor in whether a therapy happens at all. The clinical work is recognisable, but more of your day goes into affordability — formulary alternatives, assistance programmes, and the adherence conversation that follows.

Suits you if you want the counselling half of the job to matter. Ask about the patient volume, how the pharmacy interacts with the clinic alongside it, and what the dispensing workflow looks like.

Long-term care and closed-door

No walk-in counter. The work is cycle fills, cart fills, blister and bingo cards, and a verification queue driven by facility orders rather than patients at a window. Communication runs to nursing staff rather than to patients.

Suits you if you prefer production rhythm and fewer interruptions to a public-facing counter. Ask about the software — LTC systems differ meaningfully from retail dispensing systems — and the cycle schedule you would be landing in.

Home infusion and specialty

Narrower drug sets, deeper clinical work, and a documentation burden that is part of the job rather than an afterthought. Specialty and renal pharmacies run on benefits investigation, authorisation and coordination as much as on dispensing.

Suits you if you want depth over breadth. Ask about the therapeutic categories, whether sterile compounding is part of the role, and what the site expects you to document.

Compounding

Non-sterile, sterile, or both, under the site's own USP policies. The skill is real and perishable — if your cleanroom time is a few years back, say so before you are checking products rather than after.

Suits you if you have hands-on compounding experience and want to keep using it. Ask about which standards apply at that site, what you would be checking, and what the competency expectations are.

Hospital

Inpatient order verification, IV admixture, and real-time clinical back-and-forth with nursing and prescribers. The pace and the formulary are both different from any outpatient setting.

Suits you if you have inpatient experience and want to stay current in it. Ask about the shift structure, whether sterile compounding is involved, and what the handover looks like.

The questions worth asking about any of them

  • How many technicians are on with me, and is there another pharmacist on site?
  • Which dispensing system, and will someone orient me to it?
  • What does the opening hour of this shift actually look like?
  • Is this a one-off, or does the site want someone regularly?
  • What is the realistic drive, at the time of day I would be driving it?

That last one is worth being honest with yourself about. A distance that looks fine on a map is a different proposition five mornings a month.
